Hampshire Hospitals NHS Foundation Trust (HHFT) worked with CCGs to adopt a new end-of-life service to allow patients to return quickly to the comfort of their own home – significantly reducing the time spent in a hospital bed. When designing the service, HHFT needed a technology solution which could ensure maximum time with patients with minimal time on back office tasks. They chose CM2000’s CallConfirmLive! to efficiently schedule visits, manage staff availability, minimise travel between visits, ensure salaried staff are effectively utilised and evidence service delivery. Rachel Pollard, CM2000’s NHS Market Manager said: “We are delighted that CallConfirmLive! has contributed to the success of this innovative, person-centred, end-of-life service. This is a shining example of finding new ways of working to get patients closer to home, utilising technology which ensures quality and efficiency.” Dan Parr, Cancer Services Projects & Business Lead said “HHFT have improved services to our patients, reduced bed days and freed up capacity. CallConfirmLive! allows us to ensure the right people are in the right place at the right time.” A case study is now available which gives more detail about the achievements of the service and the role technology has played in the transformation.
